To ensure that your visit to our office is as convenient and efficient as possible, we are pleased to offer our HIPAA and Patient Registration forms on-line. The patient registration form may be completed electronically and printed for better legibility or completed manually.
Getting Ready for Your VisitIn order for us to provide the best care for you, it is very important that you bring the following items to your visit:
• Photo ID
• Insurance information and card(s) – *Copays will be taken at time of service
• A list of your current medications
• X-rays (CT, Sonogram, MRI, etc) film or disk and reports
• Information about what treatments, procedures and medications have been tried in the past
• Medical records from prior visits that you think may be helpful
• Download and Print the New Patient Packet- ***The Notice of Health Information Practices (HIPAA) – Please read, and keep for your records***. Sign the Acknowledgment Receipt of HTPN Notice of Health Information Practices.
• Download and Print the Patient Registation Form
